Hands-Free Accept and Reject, Without Leaving Home Row
Hands-free has a narrower honest meaning than people expect: a macropad still uses a hand, a large arcade button still uses a hand, and even voice input needs a hand to start and stop recording unless something else is doing that for you. The only approach here that is hands-free in the literal sense is a foot pedal.
What actually qualifies as hands-free
GitHub Copilot already makes accepting an inline suggestion a single keystroke, which is about as light a touch as a keyboard action gets - and it still asks something of your hand. Voice dictation typically needs a push-to-talk key to start listening, unless you are willing to leave a microphone open all the time. A macropad or a large button needs a hand to reach it, even if that hand is not the one on the mouse. Feet are the one part of you that is otherwise doing nothing during a review session.
What four pedals give you

If accept and reject really are all you need, a three-pedal unit is cheaper and covers it. And if you are not reviewing agent output hands-off very often - most of your day is spent typing anyway, hands already on the keyboard - hands-free hardware is not saving you a movement you were not making in the first place.
